New Jersey No-Fault (PIP Arbitrations)

PIP arbitrations are filed with Forthright Solutions. The items listed below with an asterisk must be provided. The other information is helpful, and should be provided.

  1. Assignment of Benefits*;
  2. HICFs (Health Insurance Claim Forms)*;
  3. EOBs (Explanation of Benefits Forms)*;
  4. Pre-certification requests with fax confirmation sheets*;
  5. The insurance carrier’s response to the pre-certification requests (denials)*;
  6. Appeal forms with fax confirmation sheets and/or certified mail return receipt card*;
  7. Affidavit of No Insurance (if applicable)*;
  8. Progress Notes, medical reports, re-examination reports, MRIs, and EMG/NCV testing results;
  9. Police Report;
  10. PIP Application;
  11. Affidavit of No Insurance; and
  12. Copy of Declaration Sheet (this is important because of “dollar-a-day policies” and limitations on PIP coverage).
